• Go to Quixtar.com (will open a new Browser Window, keep this window open too).
• Click on "Register with Quixtar" at top of the page.
• In the "I want to be a customer" section, select your country (USA or Canada). Click "Begin Registration."
• Fill in your contact info (and please enter the number 5129654 in the Referring IBO box and the first 3 letters of my last name in the Key box).  Click "create account" at the bottom. Write down your Login ID and Password to keep in a safe place.
• Then type the words "health questionnaire" in the search box at the top of the page.
• Create your Personal Profile.
• Select your name, click on the "questionnaire" button, and choose a questionnaire to fill out (there are 5 different ones offered, for example you can take the general module to start with).
• When finished, click "View Report" at the top.

After the test, you'll see what your personal nutritional needs are and will be able to order right away if you like.
